medi Epico ROMs is an elbow orthosis that immobilises the joint and then releases it in stages. An aluminium frame runs along the upper arm and forearm, a flat hinge sets the limits, and a hand rest at the far end stops the forearm rotating.

It is for elbow dislocation with associated injuries, medial or lateral epicondyle fractures, radial head fractures and distal humeral fractures — each treated with or without surgery — and for care after elbow joint replacement.

Movement is limited between 0 and 120 degrees so the range matches the stage of rehabilitation, and blocking the movements that could dislocate or partly dislocate the joint is what protects it while it heals. The hand rest prevents pronation and supination, turning the palm down and up, which is what keeps the joint between the two forearm bones still. A neck strap carries the weight of the arm, foam padding with a fluted surface presses lightly on the muscle to stimulate circulation, and the aluminium frame shapes to any arm — though not close to the hinge. medi prints that this orthosis contains semi-rigid material, and says the limits are only to be changed on your doctor's instruction.

Hand wash only, in lukewarm water (about 30°C/86°F) with a mild detergent
Close all hook-and-loop (velcro) fasteners, straps, and zippers before washing to protect the fabric
Do not use fabric softener, bleach, or harsh chemicals; do not dry clean
Remove and clean any removable pads or cushions separately with water or mild soap
Air dry fully -- do not tumble dry, use a hair dryer, or dry in direct sunlight
Apply skin creams or lotions after removing the support in the evening, not right before putting it on, since they can affect the material's properties

medi publishes these steps for putting the orthosis on. The flexion and extension limits are set on your doctor's instruction only.

Open all the hook-and-loop straps and put the orthosis on so its hinge lies directly over the bony bump on the outer side of the elbow.
Set the length of the arm rest with the two screws on the forearm splint.
Close the two forearm straps first, then the upper arm straps.
Pass one end of the neck strap through the ring on the arm cuff and fasten it.
Take the other end over your shoulder, through the ring on the hand rest, and fasten it; the neck pad should lie over the middle of the shoulder and can be trimmed with scissors if needed.

medi says that in the first few days after an injury the straps should be slackened often, so that severe swelling does not build up under them. The aluminium frame can be reshaped, but not close to the hinge.
