The Aspen® VRTX™ TLSO carries the VRTX LSO's coverage further up the back, limiting movement of the thoracic and lumbar spine after surgery, a fracture or an injury. Like the LSO it is two shells that meet at the sides, so it can be applied to a patient lying down with less rolling.

What it adds is a sternal pad on an adjustable upright at the front, held by straps that clip to the side panels and can run under the arms or over the shoulders. The pad is set about two fingers' width below the sternal notch — the dip at the base of the throat — so the upper trunk is held as well as the lower back.

Side panels pull out to the midline from inside each shell, so three sizes cover a wide range of anatomies, and an upgrade kit turns the same brace into a CTLSO.

Aspen prints this as a circumference, in inches (26 – 36, 31 – 50, 51 – 70), without naming where it is taken; the brace wraps the waist and lower trunk.

The sizes overlap because the built-in side panels extend.

Aspen prints these steps for a clinician; the brace is normally applied with the patient lying down, and a first fitting should be supervised.

Take the brace out of its packaging and find the posterior (back) assembly.
Keeping the spine in neutral alignment and with proper support, log roll the patient onto their side and centre the back assembly on the back, then roll them back onto it.
Using the handles at each side, pull the rear side panels out until they stay centred on the patient, then lift the hook off the padding on both sides.
Centre the front panel on the patient, lift the securement latch on both sides so the front side panels move freely, and pull each one out to the side midline of the body.
Press the securement latches back down to lock the front panels at that width, then attach the back panel to the front panel one side at a time.
Pull the pull tabs out and away from the body until the fit is snug, then press them down onto the loop material at the front.
Extend the sternal pad by lifting its locking mechanism and sliding it up, so the top of the pad sits about two fingers' width below the sternal notch, then lock it.
Take the clip off the side panel and clip the strap to the sternal pad on each side, then tighten by pulling down. Accessory straps can run under the arms or over the shoulders instead.
To take the brace off, park the pull tabs on the side, unclip one strap from the sternal pad, release the belt hook on that side, and reverse the steps to put it back on.
