The Aspen® Sierra™ Hyperextension TLSO is a metal-frame brace of the Jewett type: three pads — one on the breastbone, one on the pubic bone and one on the back — hold the spine in a slightly extended position so the front of the vertebrae is unloaded. It is used after compression fractures and after thoracolumbar surgery.

Unlike older Jewett frames it adjusts in both height and width, with a lever latch on the left side that opens and closes the whole brace in one movement. A pivoting pelvic bar lets the lower pad follow the wearer as they move, which is what makes an otherwise rigid frame tolerable for a full day.

Sizing is by two measurements — around the pelvis, and from the pelvis up to just below the sternal notch.

Measure the pelvic circumference around the widest part of the pelvis.

Aspen publishes these figures in inches and pounds only; the metric ones are converted.

  • Hand wash only at 30 °C (86 °F)
  • Wash with mild detergent
  • Air dry
  • Do not use bleach or other chemicals

Aspen prints these steps for a fitter; the Sierra is adjusted with a screwdriver before it is worn.

  • Set the width by removing the screws at the top of the brace, adjusting, and replacing and tightening them.
  • Set the height by removing the screws on both sides; the screws on the lever latch also need loosening or removing.
  • Set the posterior strap by peeling the hook off the loop, shortening or lengthening it at the buckle, and pressing it back down. The back pad should end up centred on the back.
  • To put the brace on, detach the clip from the frame and make sure the lever latch is open.
  • Position the brace over the torso with the top about two finger widths below the sternal notch.
  • Wrap the attachment strap around the torso, keeping the back pad centred on the back.
  • Pull the attachment clip over the post on the right side and secure it.
  • Close the lever latch on the left side to tighten the brace. To take it off, open the lever latch and detach the clip from the post.
